2型糖尿病患者合并不同程度足溃疡风险与炎症因子关系的研究

目的:探讨2型糖尿病发生足溃疡风险与炎症因子间的关系。方法:对118例2型糖尿病患者进行震动感觉阈值(VPT)测定,依据其检查结果将其分为足溃疡低风险组40例,足溃疡中度风险组38例,足溃疡高风险组40例,测定糖化血红蛋白(HbAlc)、甘油三酯(TG)、胆固醇(TC)、高密度脂蛋白-胆固醇(HDL-C)、肾功、尿微量白蛋白排泄率(UAER)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、瘦素(LEP)。结果:足溃疡高风险组的UAER、CRP、IL-6、LEP、病程较足溃疡中度风险组、足溃疡低风险组明显升高,统计学结果有显著差异;足溃疡中度风险组的UAER、CRP、IL-6、LEP、病程较足溃疡低风险组亦明显升高,与2型糖尿病发生足溃疡风险相关的因素进行Pearson相关分析结果显示足溃疡风险与年龄、病程、UAER、糖尿病视网膜病变(DR)呈强正线性相关。结论:2型糖尿病发生足溃疡风险与糖尿病病程、年龄、DR、UAER、CRP、IL-6、LEP密切相关。

The study of type 2 diabetes patients with hazard of foot ulcer and inflammatory factor

Objective:To investigate the associations between inflammatory factor and the risk factors in type 2 diabetes patients with hazard of foot ulcer. Methods: 118 type 2 diabetes were selected for this study. The patients were divided into three groups according with vibration perception threshold:40 patients with risk of were I group, 38 patients with risk of were II group, 40 patients with risk were III group. The serum levels of glycosylated hemoglobin(HbAIc),triglyceride(TG),cholesterol(TC),high density lipoprotein(HDL-C),renal function,albumin excretion rate(UAER),C-reactive protein (CRP), interleukin-6 (IL-6), Leptin (LEP) were measured with chemiluminescent immunoassay. Results:The serum levels of UAER、CRP, IL-6, LEP and course of disease in II group and III group were significantly higher than I group. In II group and I group, the serum levels of UAER、 CRP, IL-6, LEP and course of disease were significantly different, II group were significantly higher than I group; correlation analysis of hazard of food ulcer in type 2 diabetic patients with Pearson display that age, course of disease, UAER, diabetic retinopathy(DR) were positively correlated. Conclusion:The serum levels of UAER,CRP, IL-6, LEP and course of disease increased with hazard of foot ulcer in type 2 diabetic patients.
