吻合器痔上黏膜环切术治疗重度混合痔的疗效研究

目的观察吻合器痔上黏膜环切术治疗重度混合痔的疗效。方法将60例112例混合痔患者随机分成吻合器痔上黏膜环切术（PPH）组和外剥内扎（MM）组,每组各30例。观察和比较两组患者术后24h疼痛程度及术后排便疼痛指数、出血、伤口水肿、尿潴留、肛门坠胀、肛门控便能力、手术时间、住院时间和满意度等指标;随访年并记录痔复发情况。结果 PPH组术后24h疼痛指数、术后排便疼痛指数、伤口水肿、肛门控便能力、手术时间、住院时间和满意度优于MM组;两组患者在出血、尿潴留和肛门坠胀方面差异无统计学意义。随访1年,两组均有1例复发患者。结论吻合器痔上黏膜环切术是治疗混合痔的安全有效术式。

Efficacy of the Procedure for Prolapse and Hemorrhoids in the Treatment of sever Mixed Hemorrhoids

Objective To investigate the efficacy of the procedure for prolapse and hemorrhoids(PPH) in the treatment 0f sever mixed hemorrhoids.Methods 60 patients with Ⅲor IV mixed hemorrhoids admitted for surgical treatment were randomly divided into PPH group and Milligan-Morgan(MM)group averagely.Postoperative 24h pain index,pain index when defecating,postoperative urinary retention,bleeding,anal discomfort feeling,wound edema,the ability of controlling feces,operating time,hospitalization time and satisfactory degree were recorded.All the patients were followed up for 1 year.Results There were no significant differences between the 2 groups in bleeding,anal discomfort feeling and postoperative urinary retention.PPH group were better than Milligan-Morgan group in Postoperative 24h pain index,pain index when defecating,bleeding,wound edema,operating time,hospitalization time and satisfactory degree.1 patient recurred in every group.Conclusion PPH is a safe and effective treatment for sever mixed hemorrhoids.
