外伤性迟发脾破裂的手术治疗分析

目的分析外伤性迟发脾破裂的诊疗情况。方法选择外伤性迟发脾破裂96例患者,分析延迟性脾破裂的概念、诊断及外科治疗情况。结果外伤性迟发脾破裂96例经过术前病史、体格检查、诊断性腹腔穿刺、B超及CT检查而确诊;有35例行急诊脾切除术、有32例行部分脾切除术及29例行脾脏修补术,没有并发症发生而且全部治愈。结论 外伤初期极有可能伴有脾损伤,临床常常在受伤48h之后才得以确诊而导致误诊的发生。临床遇到外伤的病人必须仔细询问病史,密切观察病情变化以及进行必要的实验室检查,依据不同临床表现,选择合适的术式是临床治愈的关键。

Analysis of Delayed Traumatic Rupture Patients

Objective Delayed rupture of traumatic clinical treatment situation.Methods 96 cases of traumatic delayed rupture of spleen were retrospectively analyzed,summarized and analyzed the concept of delayed rupture of spleen,diagnosis and surgical therapy.Results 96 cases of traumatic delayed rupture of spleen after preoperative history,physical examination,diagnostic abdominal paracentesis,B ultrasound and CT examination and diagnosis;have emergency splenectomy in 35 cases,32 underwent partial splenectomy and 29 patients line spleen repair,and all healed without complications.Conclusion The early trauma associated with splenic injury is likely to clinical injury often can be diagnosed only after 48 hours,which led to the occurrence of misdiagnosis.Trauma patients in the clinical encounter must be carefully asked about the history,close observation of patient′s condition and the necessary laboratory examinations,based on the different clinical presentations,select the appropriate surgical approach is the key to clinical cure.
