| 摘 要： | 目的 总结胃代食管颈部吻合治疗食管上段癌,不同吻合方式的效果比较.方法 对210例胃代食管颈部吻合的食管癌患者采用器械吻合(68例)和手工吻合器(142例)的各项手术指标进行分析.结果 两组在吻合口狭窄的发生率、吻合时间的比较中有显著性差异(P<0.05).在手术出血量以及吻合口漏发生率方面,两组比较无明显差异(P>0.05).但手工吻合组发生3例吻合口漏,器械吻合组未发生,手术可能与器械吻合组样本量相对较小有关.结论 胃代食管颈部吻合治疗食管上段癌时使用器械吻合效果满意,可以减少吻合口并发症的发生,减少手术时间,对患者有益,值得临床推广使用.手工吻合在经济学上占优势,可根据具体情况选择.
