应变率成像定量评价冠心病局部心肌舒缩功能

摘    要:目的 观察冠心病缺血心肌应变率(SR)变化特点,探讨应变率成像(SRI)技术定量评价冠心病局部心肌功能的应用价值。方法 应用SRI对48例冠心病患者和42例正常人左室前间隔与后壁各室壁节段纵向收缩期、快速充盈期及房缩期的SR参数进行测定,并以冠脉造影结果为标准进行对比分析。结果 在收缩期和快速充盈期,冠心病缺血节段的彩色M-型SRI常表现为SRI色带变浅、消失,甚至显示为反相色彩,表明缺血心肌收缩期和快速充盈期SR减低、消失,或者呈现矛盾运动。缺血心肌收缩期和快速充盈期的径向、纵向SR测值均较对照组相应节段明显减低。冠心病组缺血节段和对照组相应节段的SRA基本无明显差异。结论 冠心病缺血心肌收缩期和快速充盈期的SR明显减低、消失,甚至倒置。SRI技术是临床无创、定量评价冠心病局部心肌功能的新方法。

Abstract:Objective To explore the clinical value of strain rate imaging (SRI) in quantitative detection of regional myocardial deformation in patients with coronary heart disease (CHD). Methods WTBZ]Forty-eight consecutive patients with CHD and forty-two normal control subjects were enrolled in the study. Radial and longitudinal SR of LV walls during systole, early diastole and atrium contraction (SRS, SRE and SR_A) were measured. Results During systolic phase and rapid filling period, color M-mode SRI showed the color in ischemic segments became lighter, disappeared, or even exhibited the antiphase color, indicating that SR of ischemic segments decreased, disappeared, or even showed inversion. Radial and longitudinal SR indexes in ischemic segments during systolic phase and rapid filling period reduced significantly than those in corresponding normal segments of controls. On the whole, no significant differences were found in SR_A between ischemic segments of CHD and corresponding normal segments of controls. Conclusion SRI can be used as a new technique for evaluating regional myocardial function noninvasively and quantitatively.
