四种幽门螺杆菌感染根除治疗方案的疗效分析

目的:观察四种幽门螺旋杆菌(Hp)根除治疗方案的疗效。方法:200例经胃窦黏膜病理组织学及14C-尿素呼气试验(14C-UBT)均呈阳性者诊断为Hp感染,随机将这些患者分为A、B、C、D四个治疗组:A组为奥美拉唑(20mg)、阿莫西林(1 000mg)、克拉霉素(500mg)和果胶铋(200mg);B组为奥美拉唑(20mg)、阿莫西林(1 000mg)、左氧氟沙星片(200mg)和果胶铋(200mg),C组为奥美拉唑(20mg)、阿莫西林(1 000mg)、呋喃唑酮(200mg)和果胶铋(200mg);D组为奥美拉唑(20mg)、左氧氟沙星片(200mg)、呋喃唑酮(200mg)和果胶铋(200mg);均为口服,每日2次,疗程7天,疗程结束停药4周后复查14C-UBT,阴性为Hp根除,并对各组的疗效进行分析。结果:A、B、C和D组Hp根除率分别79.2%、85.1%、91.8%和93.7%,C组和D组明显高于A组和B组(P<0.05),D组高于C组,但差异无统计学意义(P>0.05)。结论:含呋喃唑酮的Hp根除治疗方案根除率高,尤以与左氧氟沙星联用时效果更明显。

Analysis of the Effects of Four Eradication Treatment Programs in Helicobacter Pylori Infection

Objective:To observe the effects of four eradication treatment programs in Helicobacter pylori(Hp) infection.Methods:200 patients diagnosed with gastric mucosa biopsy histology and 14C-urea breath test(14C-UBT) as Hp infection were randomly divided into four groups.Group A were taken orally with omeprazole(20mg),Amoxicillin(1 000mg),clarithromycin(500mg) and pectin bismuth(200mg),Group B were taken orally with omeprazole(20mg)、amoxicillin(1 000mg)、levofloxacin(200mg) and pectin bismuth(200mg),Group C were taken orally with omeprazole(20mg),amoxicillin(1000mg),furazolidone(200mg) and pectin bismuth(200mg),Group D were taken orally with omeprazole(20mg),levofloxacin(200mg),furazolidone(200mg) and pectin bismuth(200mg),bid for 7 days.After stopping treatment for 4 weeks,they were taken with 14C-UBT and confirmed as the Hp eradication for the negative results.The treatment effects of each group were analyzed.Results:The ratio of Hp eradication in Group A,B,C and D was 79.2%,85.1%,91.8% and 93.7%,respectively,which in Group C and D was higher than in Group A and B(P<0.05).The difference in Group C and D was not stastically significant(P>0.05).Conclusion:The ratio of Hp eradication in treatment program with furazolidone is the higher,The effect of furazolidone combined with levofloxacin is better.
