MRI及1H-MRS对原发性颅内B细胞淋巴瘤的诊断价值

目的:探讨免疫功能正常人原发性颅内B细胞淋巴瘤常规MRI及多体素质子磁共振波谱成像(1H-MRS)表现。方法:回顾性分析25例经病理证实的原发性颅内B细胞淋巴瘤的常规MRI和1H-MRS表现。结果:25例患者共检出36个病灶,其中17例单发,8例多发;平扫T1WI呈稍低信号,T2WI呈等或稍高信号;增强扫描肿瘤呈明显均匀强化;肿瘤占位效应多为轻或中度;"尖角征"和"缺口征"是增强后最具特异性的征象;胆碱(Cho)峰升高、氮-乙酰天门冬氨酸(NAA)降低、脂质(Lip)峰高耸是1H-MRS主要表现。结论:原发性颅内B细胞淋巴瘤MRI、1H-MRS具有一定的特征,联合应用可提高MRI对肿瘤的诊断水平。

Diagnostic Value of MRI and MR Spectrum for Primary Intracranial B-cell Lymphoma

Objective:To retrospectively analyze conventional MR imaging and 1H-MR spectroscopy(1 H-MRS)features of primary intracranial lymphoma in patients with normal immunological function.Methods:The MRI and 1 H-MRS features were retrospectively analyzed in 25 cases of B-cell primary intracranial lymphoma proved by operation and pathology,while 1 H-MRS features of 19 cases were retrospectively studied.Results:36 nodules were detected in all 25 patients,The lesions showed isointensity on TlWI,isointensity or slightly hyperintensity on T2WI,and had mild or moderate edema.Most lesions demonstrated "gap sign","cusp sign" and obvious Enhancement was intense and homogenous,the presence of "incision sign" and "angular sign" were more specific.1H-MRS showed decreased NAA and elevated choline(Cho) with large lipid peak.Conclusion:Combining with the conventional MRI appearance with1 H-MRS features can improve MRI diagnostic accuracy of primary intracranial lymphoma.
