阿托品联合盐酸戊乙奎醚治疗有机磷农药中毒分析

摘    要:目的对比性研究联合应用阿托品和盐酸戊乙奎醚(联合组)与单独应用阿托品(对照组)在治疗急性有机磷农药中毒的临床疗效。方法回顾性调查分析2007年1月-2010年12月大庆油田总医院收治的87例急性有机磷农药中毒患者,根据治疗方式不同分为联合组和对照组。联合组早期应用阿托品达阿托品化后停用阿托品,开始应用盐酸戊乙奎醚治疗直至病情缓解,对照组只应用阿托品治疗。观察治愈率、病死率、中毒症状持续时间、胆碱酯酶恢复时间、药物不良反应发生率、住院天数等指标。结果联合组43例患者死亡1例,对照组44例患者死亡4例。联合组与对照组相比,联合组具有治愈率提高(P<0.05)、病死率降低(P<0.05)、胆碱酯酶恢复时间缩短(P<0.01)、中毒症状持续时间缩短(P<0.01)、药物不良反应发生率减少(P<0.01)、住院天数减少(P<0.01)等优点。结论阿托品联合盐酸戊乙奎醚治疗急性有机磷农药中毒临床疗效优于单独应用阿托品治疗,值得在临床推广。

Effect of Atropine Combination with Hydrochloride Penehyclidine on Organophosphate Pesticide Poisoning

Abstract:Objective A comparative study was conducted to evaluate the clinical efficacy of atropine combined with hydrochloride penehyclidine(Combination group) and atropine only(control group) in the treatment of acute organophosphorus pesticide poisoning.Methods The clinical data of 87 patients with acute organophosphorus pesticide poisoning treated in our hospital from January 2007 to December 2010 were analyzed retrospectively.The patients were divided into control group and combination group according to the treatments.The control group was treated with atropine and the combination group with atropine in early phase until atropinization,and then followed by hydrochloric acid penehyclidine until the disease remission.The symptoms of poisoning duration,cholinesterase recovery time,cure rates,fatality rate,adverse drug reaction rates,hospitalization delay and other indicators were recorded.Results One in 43 cases of the combination group died,4 in 44 cases of the control group died.In the combination group,the cure rate increased(P<0.05),fatality rate descended(P<0.05),the duration of symptoms of poisoning reduces(P<0.01),cholinesterase recovery time reduces(P<0.01),the incidence of adverse drug reactions decreased(P<0.01),hospitalization delay reduced(P<0.01).Conclusion The hydrochloric acid penehyclidine combined with atropine for the acute organophosphorus pesticide poisoning exhibit superior efficacy to atropine alone,and worth to generalized in clinic.
