儿童CVA诱发因素的临床调查及辨证分析

目的探究咳嗽变异型哮喘的发病因素并对之进行辨证分析。方法本课题组对2008年以来山东省烟台市中医院327例CVA患儿进行抽样调查研究,调查CVA患儿的相关发病因素。整个调查工作由接诊中医大夫采用望、闻、问、切的方式进行,患儿不能表述的,由家长陪同填写"儿童CVA发作诱因调查表"。结果发病与首发年龄、发作时间、天气变化、感冒、剧烈运动、冷刺激、吸烟、花粉、体质、家族史等密切相关。结论 CVA发病是由于肺、脾、肾三脏虚损导致易受外邪、易生痰邪、易助食滞。这与中医理论认为小儿多具有肺脾肾亏虚之特点相吻合。在其缓解期,应该发挥中医治未病的优势,未雨绸缪。缓解期扶正固本是预防复发的关键,病变脏腑在肺、脾、肾,病理因素有虚、痰饮、食积等。

Clinical Survey and Dialectical Analysis on Induced Factor of Cough Variant Asthma in Children

Objective To investigate the risk factors related to cough variant asthma(CVA) and form the dialectical analysis.Methods We conducted a sample survey of 327 children with CVA from Yantai Hospital of Traditional Chinese Medicine,the risk factors related to CVA were investigated.The "Questionnaire of CVA Morbidity Factor" was finished by the reception doctor of TCM through observation,listening,interrogation and pulse-taking.For those children who cannot be expressed,the questionnaire was finished by their parents.Results Incidence rate of CVA was closely related to age of onset,duration of episodes,weather,cold,strenuous exercise,cold stimulation,smoking,pollen,physique,family history and others.Conclusion From the differential of TCM,we should prevent the disease from lung,spleen,kidney,which related to exogenous pathogens,phlegm evil and food stagnation,which is coincide with the theory of TCM in pediatric multiple with lung spleen and kidney deficiency of characteristics.During their remission period,we should follow the prognosis of a disease.Support the Healthy Energy is critical in lung,spleen,kidney,and pathological factors including deficiency,phlegm and dyspepsia during remission.
