参与式方法在社区糖尿病患者健康管理中的应用

目的探讨参与式方法在社区糖尿病患者健康管理中的应用及效果,为社区糖尿病患者健康管理模式的发展提供参考。方法将抽取的4个社区分为两组,对第1组(参与管理组)社区内的糖尿病患者进行参与式健康管理,第2组(自我管理组)社区内的糖尿病患者进行自我管理式的健康管理,在干预前及干预后的3个月、6个月、9个月、12个月进行血糖测量,比较两组患者的血糖控制率及干预后患者管理的自我效能。结果参与管理组的每月时间成本为8h,自我管理组的每月时间成本为16 h。干预1年后,参与管理组患者管理效能平均得分为141.65,与自我管理组的123.83差异有统计学意义(P<0.05)。两组管理的血糖控制率分别为79.44%和83.84%,差异无统计学意义(P>0.05)。结论参与式方法是一种适合我国目前全科医生紧缺环境的社区糖尿病患者健康管理的有效模式。

Application of Participatory Approach on Health Management of Diabetes

Objective To explore the effect of participatory approach on health management of diabetes,and to provide reference for the development of health management mode of diabetes.Methods Four communities were divided into two groups,the diabetes in group one were managed with participatory approach,and the diabetes in group two were managed with self-management approach.Blood glucose testing was carried out for all diabetes,after 3 months,6 months,9 months,and 12 months of the study.The control rate of blood glucose and diabetes management self-efficacy were compared between the two groups.Results The monthly time cost of two groups were 8 and 16 hours respectively.After one year of the study,the average score of diabetes management self-efficacy of the diabetes in group one was significantly higher than group two(141.65 vs 123.83),and there was no significant difference of control rate of blood glucose between two groups(79.44% vs 83.84%).Conclusion The participatory approach was an effective mode of health management of diabetes,which was fit for the era of general practitioner shortage.
