冷冻联合光动力疗法治疗尿道口尖锐湿疣

目的观察应用冷冻联合5-氨基酮戊酸(ALA)光动力疗法治疗尿道口尖锐湿疣的疗效。方法尿道口尖锐湿疣患者90例,随机分为3组,每组30例。第一组为冷冻治疗组,冷冻疣体。第二组为光动力疗法治疗组,皮损区涂以20%ALA乳膏后3 h,以XD-635AB型光动力激光治疗仪照射,距离5～10 cm,照射时间20 min,光斑直径1～4 cm,能量密度100 J/cm2。间隔7～10 d治疗1次,共治疗3次。第三组为冷冻联合光动力疗法治疗组,冷冻疣体后立即行光动力疗法治疗。末次治疗后第1周进行疗效评价,治疗结束后随访6个月。结果 3组患者治疗后疣体均完全消失,黏膜恢复正常。第一组复发率为53.3%,平均复发时间为24.5 d;第二组复发率为13.3%,平均复发天数为59.5 d;第三组复发率为6.7%,平均复发天数为95 d。结论 5-ALA光动力疗法能有效治疗尿道口尖锐湿疣,联合冷冻治疗,具有安全,复发率低的特点。

Liquid Nitrogen Freezing Combined with 5-aminolaevulinic Acid Photodynamic Therapy for Urethral Condyloma Acuminatum

Objective To investigate the efficacy of liquid nitrogen freezing combined with 5-aminolaevulinic acid photodynamic therapy(ALA-PDT) in the treatment of urethral condyloma acuminatum.Methods Total 90 patients with urethral condyloma acuminatum were randomly assigned to the liquid nitrogen freezing group,photodynamic therapy group and liquid nitrogen freezing-photodynamic therapy group.The treatment was repeated weekly for 3 times.The primary efficacy endpoint was the warts clearance rate 1week after the last treatment.The recurrence rate was evaluated for continuous 6 months.Results The warts were all disappeared after treatment in all three groups and the mucous membranes were all normal.The recurrence rate of the 30 patients solely treated by liquid nitrogen freezing was 53.3%,and their average recurrence intervals were 24.5 days.The recurrence rate of ALA-PDT was 13.3% and their average recurrence intervals were 59.5 days.The recurrence rate of liquid nitrogen freezing combined with ALA-photodynamic therapy was 6.7% and their average recurrence intervals were 95 days.Conclusion The liquid nitrogen freezing combined with ALA-photodynamic therapy is a very safe and very low recurrence for the urethral condyloma acuminatum.
