彩超对糖尿病肾血流动力学改变的观察分析

目的观察糖尿病(DM)肾动脉的血流动力学的改变,探讨彩色多普勒超声诊断糖尿病肾病(DN)的临床价值。方法应用彩色多普勒血流显像对45例2型糖尿病肾病(DN)患者的段动脉(SRA)、叶间动脉(IRA)、弓形动脉(ARA)进行检查,并对其血流的收缩期最大血流速度(Vsmax)、舒张期最低血流速度(Vdmin)和搏动指数(PI)、阻力指数(RI)进行对比分析。结果随着肾脏的损害,患者肾血流多普勒频谱特点是:SRA、IRA、ARA的Vsmax、Vdmin,PI、RI增高。尤其当RI大于0.7,肾脏将出现早期损害。结论彩色多普勒超声检测肾血流参数在预测糖尿病肾病肾脏损害方面有一定临床参考价值,为糖尿病肾病的诊断提供了一种简便的方法。

Observation on Renal Hemodynamics in Patients with Diabetic Nephropathy by Color Flow Doppler Ultrasound

Objective To explore the renal artery hemodynamics indexes in patients with diabetes nephropathy(DN) and the value of the color Doppler ultrasonography in the diagnosis of DN.Methods Fifty five patients with type II diabetes nephropathy were examined by color Doppler ultrasonography.The systolic velocity(Vsmax),diastolic velocity(Vdmin) and Pulse Index(PI),resistance index(RI) of segmental renal arteries(SRA),interlobular renal arteries(IRA) and arcuate artery(ARA) were evaluated.Results Following the process of renal dysfunction,the Vsmax and the Vdmin of renal arterial flow decreased.PI,RI of SRA,IRA and ARA increased significantly.RI over 0.7 indicated the injured renal function.Conclusion Color Doppler ultrasound renal blood flow parameter has a certain reference value in predicting renal lesions of diabetic nephropathy;it provides a simple method in diagnosis of diabetic nephropathy.
