糖尿病患者社区干预的效果分析

目的对糖尿病患者进行为期一年的社区干预,评价干预的效果,寻求糖尿病有效的干预措施。方法对本辖区社区1572位45岁以上人群进行健康普查,检出符合2010年中国糖尿病社区干预指南诊断标准的患者126人,进行跟踪随访,有组织、有计划地进行社区健康教育和干预1年观察效果。结果经过社区卫生服务综合干预后,糖尿病患者危险因素知晓率明显升高(P<0.01),生活方式、行为及防治的依从性显著升高(P<0.01)。干预前后血糖及其他生化指标统计学分析差异有统计学意义。结论通过社区卫生服务平台综合干预,寻找出群众乐意接受有效的措施,能显著提高糖尿病的防治效果。

The Analysis of Effect of Diabetes of Community Intervention

Objective To assess intervention effects,and seek effective diabetes interventions of prevention and treatment of diabetic patients in the community health center.Methods Health survey was conducted among 1572 people over age of 45 in this community,126 cases of diabetic patients were detected for the study.Community health education and intervention were organized and planned in a year,the effect was observed.Results After a comprehensive community health service intervention,awareness was significantly higher in patients with risk factors(P<0.01),lifestyle,behavior,and prevention compliance was significantly higher(P<0.01).Conclusion Comprehensive intervention through the community health services platform,looking out to the masses willing to accept effective measures,which can significantly improve diabetes prevention and treatment effect.
