基于区域医疗联合体的慢性病健康管理

近些年来,随着中国社会和经济的快速发展,人群在生活方式和疾病模式上发生了巨大转变,慢性病患者的数量与日俱增,以至于慢性疾病逐渐成为中国人群的最主要死因。慢性疾病病程长,可以治疗却不可以治愈,因而长期、持续的医疗卫生服务对于慢性病管理极其重要。本文通过对我国慢性病的流行病现状、慢性病的管理现状以及慢性病的防治现状进行深入剖析,梳理总结了我国的社区慢性病管理在现阶段的政府政策、人群认知、管理模式、管理实践以及人才建设等方面存在的主要问题和弊端。同时在上海市建立区域性医疗联合体的大背景下,本文对区域性医疗联合体的优势特征进行研究,并在区域性医疗联合体的建立背景下对慢性病在社区管理、防治和干预的前景进行进一步地分析和探讨。

Chronic Disease Management Based on the Regional Medical Association

In the recent years,with the rapid development of society and economy,great changes have taken place in people’s lifestyles and disease model.The number of patients with the chronic diseases is increasing,so that the chronic disease has gradually become the primary cause of death in China.Because of the long course,the chronic diseases can be treated but not cured,which presents the significance of long-term and continuing medical service in chronic disease management.By the analysis of the epidemic,management,prevention and cure of chronic diseases in China,primal issues are presented in the management of chronic diseases in the community on the stage,for instance,in policy,people cognition,management model and personnel construction.Under the background of building the Regional Medical Associations in Shanghai,the models and prospect of chronic diseases management should be further analyzed and discussed based on the superiorities of the Regional Medical Associations.
