慢性肺心病患者的心理障碍及其影响因素的分析

目的探讨慢性肺心病患者心理障碍状况以及导致心理障碍的主要影响因素。方法采用抑郁自评量表(Self Rating Depression Scale,SDS)和焦虑自评量表(Self Rating Anxiety Scale,SAS)对80例慢性肺心病患者(男性50例,女性30例)抑郁和焦虑状况进行评价。结果慢性肺心病患者SDS、SAS总平均分与正常组比较明显增高,差异有统计学意义(P<0.01),其中女性较男性更明显(P<0.05)。慢性肺心病患者PaO2高于60 mm Hg,与PaO2小于60 mm Hg比较,两组间SDS、SAS评分比较,差异有统计学意义(P<0.001);PaCO2高于50 mm Hg组与PaCO2小于50 mm Hg组比较,差异有统计学意义(P<0.001)。结论慢性肺心病患者存在心理障碍,与低氧血症和高碳酸血症有关,心理辅导治疗应作为慢性肺心病患者综合治疗的重要内容。

Analysis of Depression and Anxiety in Patients with Chronic Cor Pulmonale and Their Influencing Factors

Objective To investigate the information of depression and anxiety in patients with chronic cor pulmonale(CCP) and their main influencing factors.Methods Two standardized scales "Self Rating Depression Scale,SDS" and "Self Rating Anxiety Scale,SAS" were performed for the survey in the study.80 cases(50 males,30 females) with CCP and 30 cases with control group were investigated.Results The total average marks of SDS and SAS on CCP were significantly higher than those of control group(P<0.01).Females’total average marks in patients were higher than that on males’in patients(P<0.05).The marks patients with PaO2 less than 60 mm Hg were significantly higher than those of patients with PaO2 more than 60 mm Hg(P<0.001).Moreover,the marks of patients with PaCO2 more than 50 mm Hg were significantly higher than those of patients with PaCO2 less than 50 mm Hg(P<0.001).Conclusion Patients with CCP had got depression and anxiety,which was relevant to hypoxemia and hypercapnia.Psychological therapy should be conducted as a very important part in the treatment of CCP patients.
