多胎合并婴儿肝炎综合征的临床特点及转归

 目的 探讨多胎合并婴儿肝炎综合征（IHS）的病因、临床发病特点、相关治疗及疾病转归。方法 对43例多胎合并IHS患儿的临床资料进行回顾性病因和预后分析。结果高危因素分析：43例合并IHS多胎患儿与其未患IHS的同胞相比，小于胎龄儿和多胎之小显著增多（P＜0.01）、并在建立稳定肠道营养时间（P＜0.01）、静脉营养的使用时间（P＜0.005）及静脉营养最大量（P＜0.05）上差异有统计学意义。治疗及转归：经综合治疗后，近期有效率60.47%。远期有效率100%。结论 宫内发育迟缓、多胎之小、肠道营养建立延迟及静脉营养的长期、大量应用可能是导致多胎合并IHS的重要原因，此类患儿经综合治疗后，一般预后较好。

Clinical Characteristics and Outcomes of Multiple Fetuses with Infantile Hepatitis Syndrome

Objective To explore the etiology,clinical characteristics,related treatment and outcomes of multiple fetuses with infantile hepatitis syndrome(IHS).Methods The data of multiple fetuses with IHS in NICU from October 2004 to February 2008 were reviewed for causes and prognosis.Results High-risk factors analysis:Compared with their siblings without HIS,significant differences were noted in weight related gestation age and birth sequence(P < 0.01),beginning of stable Intestinal nutrition(P < 0.01),and the length(P < 0.005) and the maximum(P < 0.05) of parenteral nutrition in 43 cases of multiple fetuses with HIS.Treatment and prognosis:The recent effective rate was 60.47%,and the long-term′s was 100%.Conclusion Intrauterine retardation,later birth of multiple fetuses,delayed intestinal nutrition,prolonged application of massive parenteral nutrition are the main causes of multiple fetuses with IHS,and the prognosis is good with multimodality therapy.
