HBsAg和HBeAg临界值指数与PreS1-Ag相关性及应用价值的探讨

目的分析HBsAg和HBeAg临界值指数(COI)的相关性及其与PreS1-Ag阳性率的关系,为探讨有效的HBV筛查和检测提供依据。方法电化学发光法检测500例HBsAg阳性标本,测其HBsAg和HBeAg的COI,按照HBeAg的阴阳性及HBsAg COI的大小进行分组,采用卡方检验分别分析比较HBsAg COI和HBeAg COI大小的关系及其与PreS1-Ag的相关性。结果 HBeAg与HBsAg的COI大小呈负相关,差异有统计学意义(χ2=20.788,P<0.05);HBeAg阳性组,HBsAg的COI大小与PreS1-Ag差异无统计学意义;HBeAg阴性组,HBsAg的COI<1000组PreS1-Ag阳性率明显低于COI>1000组(χ2=55.816,P<0.05);HBeAg阳性组与阴性组PreS1Ag阳性率差异有统计学意义(χ2=40.28,P<0.05)。结论 HBeAg与PreS1-Ag有很好的相关性,可作为急性乙型肝炎辅助诊断和预后的指标,当HBeAg阴性时,HBsAg的COI可以作为HBsAg含量的初步判断指标,且在一定范围内,COI高可以作为乙型肝炎感染急性期的一个指征。

Correlation of HBsAg and HBeAg with PreS1-Ag and its application value

OBJECTIVE To provide evidence for effective filtration and detection of HBV by analyzing the relationship between the COI of HBsAg and HBeAg and the positive rate of PreS1-Ag.METHODS A total of 500 HBsAg positive samples were detected by ECLIA,the COI of HBsAg and HBeAg were determined and grouped according to the value of COI of HBsAg and HBeAg,and their correlations with the positive rate of PreS1-Ag were analyzed by χ2 test.RESULTS There was a negative correlation between the COI of HBeAg and HBsAg,the difference was statistically significant(χ2=20.788,P<0.05).In the HBeAg positive group,there was no significant difference between the value of HBsAg and the positive rate of PreS1-Ag;in HBeAg negative group,the positive rate of PreS1-Ag in HBsAg group with COI<1000 was significantly lower than that in HBsAg group with COI>1000(χ2=55.816,P<0.05).The difference in the positive rate of PreS1Ag between HBeAg positive and negative group(x2=40.28,P<0.05) was significant.CONCLUSION HBeAg and PreS1-Ag are well correlated,which is a good index for the diagnosis and prognosis of acute hepatitis B.If the HBe-Ag is negative,the COT of HBsAg can be used for initial judgment of the content of HBsAg,that is to say that the high COI of HBsAg is an indicator of HBV in the acute phase.
