| 摘 要： | 目的::分析脊椎骨折X线平片与螺旋CT表现,探讨它们对脊椎骨折的诊断价值。方法:回顾分析156例脊椎骨折的X线平片与螺旋CT影像学资料。结果:156例脊椎骨折中,颈椎骨折45例,胸椎骨折22例,腰椎骨折79例,骶尾椎10例;其中颈胸椎骨折6例,胸腰椎骨折15例。X线平片显示椎体骨折、附件骨折、骨碎片、小关节滑脱、椎管狭窄、脊柱后突成角及椎体移位检出率分别为97.44%(152例)、22.44%(35例)、19.87%(31例)、16.03%(25例)、19.87%(31例)、27.56%(43例)、14.74%(23例);CT分别为100%(156例)、33.97%(53例)、29.49%(46例)、26.64%(40例)、26.64%(40例)、32.05%(50例)、26.92%(42例)。通过χ2检验比较两种检查方法:椎体骨折P>0.05;附件骨折脱位P<0.05;骨碎片P<0.05;椎管狭窄变形P<0.05。CT可详细显示爆裂型骨折、脊髓受压、血肿等软组织改变。结论:X线是脊椎骨折首选、基本诊断方法。螺旋CT及三维、多平面重建可提供比X线更多的信息,减少X线平片的漏诊。
