帕金森病立体定向治疗并发症的防治

目的　探讨帕金森病立体定向治疗并发症的原因及防治。方法　我院 1 998年 4月至 2 0 0 0年 4月两年内立体定向治疗帕金森病人 537例 ,并进行随访一年以上 ,对这些临床资料进行了回顾性总结。结果　 573例手术病人 ,术后震颤、僵直消除或基本消除的改善率为 98.6 % (565/ 573) ;运动迟缓改善率为 90 .0 % (51 6/ 573)。生活质量均明显改善。改善不明显率仅为 0 .52 % (3/ 573)。手术并发症低 ,恢复良好。结论　帕金森病立体定向手术强调解剖、电生理双重定位 ,结合手术方式设计完善的手术入路 ,严格把握手术适应证 ,适当术前训练 ,正确处理术中并发症 ,可明显提高疗效 ,降低并发症。深部脑电刺激器 (DBS)在减少并发症方面应用前景广阔

Prevent and treat the complication of stereotactic operation for Parkinson' s disease

Objective To explore the prevention and treatment the complication of stereotactic operation for Parkinson's disease.Methods The clinical data of 573 cases and follow up over 1 year after stereotactic operation for Parkinson's disease from Apr. 1998 to Apr.2000 were analyzed retrospectively.Result Obvious relief of tremor and rigidity was 98.6% (565/573),distinct relief of akinesia was 90.0%(516/573),which improved the life quality of post operation patient clearly.Unobvious relief rate was only 0.52%(3/573).Surgical complication occurred rarely.Conclusion It was emphasized to combine the anatomic location and electrophysiological confirmation,to design perfect surgical approach according to the surgical manner,to be strict on surgical indication, to train the patient before operation so as to cooperate during the surgery,and to deal with the complication properly during operation.All of these above measure can improve the surgical efficiency distinctly,minimize the complication.Deep brain stimulator(DBS) will have the prospect of a very wide use to minimize the complication in the future.
