纤维支气管镜辅助治疗重症呼吸机相关性肺炎的疗效

摘    要:目的:分析纤维支气管镜辅助治疗重症呼吸机相关性肺炎的疗效。方法:选取2017年6月~2019年4月本院收治的重症呼吸机相关性肺炎患者80例为研究对象,将其按照随机方式平均分为两组,对照组40例患者采用常规治疗,观察组40例患者在常规治疗基础上采用纤维支气管镜辅助治疗,观察两组患者治疗前后的IL-6和CRP水平以及治疗有效率。结果:治疗前两组患者的IL-6和CRP水平差异无统计学意义(P>0.05),治疗后观察组患者的IL-6和CRP水平分别为(101.21±10.88)pg/mL和(35.82±5.43)mg/L,均低于对照组患者的(116.78±10.35)pg/mL和(43.31±5.76)mg/L,差异有统计学意义(P<0.05)。观察组患者中治疗显效患者25例,治疗有效患者13例,总治疗有效率为95.00%;对照组患者中治疗显效患者18例,治疗有效患者13例,总治疗有效率为77.50%,组间差异有统计学意义(P<0.05)。结论:对于重症呼吸机相关性肺炎患者采用纤维支气管镜辅助治疗有助于降低患者的炎症因子水平,提高治疗有效率。

Abstract:Objective:To analyze the efficacy of fiberoptic bronchoscopy in the treatment of severe ventilator-associated pneumonia.Methods:Eighty patients with severe ventilator-associated pneumonia treated in our hospital from June 2017 to April 2019 were selected as the research subjects,and they were divided into two groups according to randomization.The control group of 40 patients were treated with conventional treatment.Forty patients in the group were treated with fiberoptic bronchoscopy on the basis of conventional treatment.The levels of IL-6 and CRP before and after treatment and the effectiveness of the treatment were observed.Results:There was no significant difference in IL-6 and CRP levels between the two groups of patients before treatment(P>0.05).After treatment,the levels of IL-6 and CRP in the test group were(101.21±10.88)pg/mL and(35.82±5.43)mg/L,which were lower than(116.78±10.35)pg/mL and(43.31±5.76)mg/L in patients in the control group,and the difference was statistically significant(P<0.05).Among the patients in the experimental group,25 cases were markedly effective,13 cases were effectively treated,and the total effective rate was 95.00%;in the control group,18 cases were markedly effective,and 13 cases were effectively treated.The total effective rate was 77.50%.The difference was statistically significant(P<0.05).Conclusion:Adopting fiberoptic bronchoscopy for patients with severe ventilator-associated pneumonia can help reduce the level of inflammatory factors in patients and improve the treatment efficiency.
