针形切开刀在治疗性ERCP中的应用

目的探讨使用针形切开刀在插管困难时对ERCP成功率的作用及安全性。方法分析58例经针形切开刀预切术后再行治疗性ERCP患者的临床资料。结果58例行治疗性ERCP病例,常规插管无法完成胆管插管,经针形切开刀预切开后胆管插管成功54例(成功率93.1%)。术后并发胰腺炎6例,上消化道出血2例均保守治愈。结论在常规插管失败后,利用针形切开刀预切开乳头括约肌可提高插管成功率,术后并发症无明显增多。

Application of Acicular Cuts Surgery in the Treatment of ERCP

Objective To explore the successful ratio and the saiety oi ERCP with aeicular scalpel surgery for difficult intubation. Method Analyses the clinical data about 58 eases with the acieular cut-incision surgery in ERCP treatment. Results Among 58 ERCP routine treatment of patients, conventional intubation unable to complete bile duct intubation, the successful eases of bile duct intubalion after acieular cut-incision surgery is 54 （93. 1% success rate）. 6 cases of postoperative complication panerealitis, 2 eases of upper gastrointestinal bleeding are treated conservatively. Conclusion After the failure of conventional intubation, the use of aeicular cuts to sphineterotomy surgery can improve the success rate of intubation, has no significant increase in postoperative comolications.
