持续性非瓣膜心房颤动患者心电图f波特点与血栓栓塞事件的关系

摘    要:目的:探讨持续性非瓣膜心房颤动患者心电图f波特点与血栓栓塞事件的关系,从而预测血栓栓塞事件的发生。方法:56例持续性非瓣膜心房颤动患者分为两组:血栓栓塞阳性组(12例)和血栓栓塞阴性组(44例)。测量体表心电图V1导联f波振幅、时限和f-f间期,对结果进行统计学比较。结果:血栓栓塞阳性组f波振幅明显低于血栓栓塞阴性组(0.08±0.04vs0.10±0.02,P<0.05),f-f间期短于血栓栓塞阴性组(0.10±0.02vs0.13±0.02,P<0.05),f波时限两组比较无显著差异(0.09±0.02vs0.08±0.02,P>0.05),但房颤病程与f波时限之间有直线回归关系,随病程的延长,f波时限愈长。结论:临床上可利用体表心电图f波振幅和f-f间期的特点,预测非瓣膜性心房颤动患者血栓栓塞的危险性。

Abstract:Objective:To explore the relationship between the characteristics of fibrillory wave and thromboembolism events in patients with chronic non-valvular atrial fibrillation(NVAF) and predict thromboembolism events.Methods: 56 patients with NVAF were divided into 2 groups: thromboembolism group(n=12) and non-thromboembolism group(n=44).We measured the amplitude、timing of fibrillory wave and f-f interval,then made a statistics analysis for the results.Results: The amplitude of fibrillory wave in thromboembolism group is obviously lower than that in non-thromboembolism group(0.08±0.04vs0.10±0.02,P<0.05).The f-f interval of thromboembolism group is shorter than that of non-thromboembolism group(0.10±0.02vs0.13±0.02,P<0.05),and there is no significant difference in the timing of fibrillory wave between two groups(0.09±0.02vs0.08±0.02,P=0.27),but there is a linear regression relationship between the history of atrial fribrillation and the timing of fibrillory wave.The longer the history,the longer the timing of fibrillory wave.Conclusion: We can use the characteristics of the amplitude and the timing of fibrillory wave to predict the risk of thromboembolism events.
