大肠黑变病59例临床与内镜分析

目的:探讨大肠黑变病的病因及内镜特点.方法:总结我院2000～2005年间电子结肠镜检查4 439例所发现的59例大肠黑变病,并进行回顾性分析.结果:所有病例均为首次确诊.在59例大肠黑变病中,年龄大于60岁者占50.85 %,便秘占69.49%,长期服用泻剂者占62.71%;一次或多次结肠镜复查共5例,其中4例大肠黑变病病变消失时间为6个月、1年、1年10个月及2年.全部病例均经病理组织学检查证实,伴单发或多发性息肉13例,其中腺瘤性息肉10例.结论:病因主要是因便秘而长期服用蒽醌类泻剂;本病多见于老年人;内镜下具有独特表现;是一种良性可逆疾病;常合并具癌变倾向的腺瘤型息肉.

Clinical and endoscopic features in 59 patients with colon melanosis

Objective: To investigate the etiology and endoscopic features of colon melanosis.Methods: 59 patients with colorectal melanosis out of 4 439 case were found by video endoscopic examination in our hospital from 2000 to 2005 year and were retrospectively analyzed.Results: All the patients were diagnosed for the first time.In 59 patients with colorectal melanosis,older than 60 years accounted for 50.85% and 69.49% of them had constipation,62.71% of them took laxatives for long time.Five cases were reviewed by one or more times colonoscopy tests including 4 cases of colorectal melanosis with disappearing for six months,1 year,1 year and 10 months and 2 years.All the cases were confirmed by histopathologic examination,including 13 cases of patients with single or multiple polyps and 10 cases of adenomatous polyposis.Conclusion: The major causes of colon melanosis are chronic constipation and taking anthraquinone laxatives.The disease prevails in the people with old age and it has endoscopic unique clinical features of colonoscopy.It is a benign disease which is often associated with carcinogenesis adenomatous polyps.
