膀胱PUNLMP和低分级乳头状癌的生物学比较

目的:探讨膀胱低度恶性潜能的乳头状尿路上皮肿瘤(papillary urothelial neoplasms of low malignant potential,PUNLMP)和低分级乳头状癌low-grade(grade1)papillary carcinomas,LG]的生物学差异。方法:统计两种肿瘤的2年复发率,并利用免疫组化的方法比较34例膀胱PUNLMP和46例LG的细胞增殖指标(Ki-67)和癌基因/抑癌基因(c-erbB-2、p53)的阳性表达率。结果:PUN-LMP的2年复发率及Ki-67和p53的阳性表达率低于LG,P值分别为0.020、0.001和0.003;而c-erbB-2的阳性表达率差异无统计学意义。结论:膀胱PUNLMP和LG在生物学行为和临床预后上有较明显的差异,但PUNLMP仍需要长期随访。

Biologic differences between papillary urothelial neoplasms of low malignant potential and low-grade papillary carcinomas of the bladder

OBJECTIVE:To study the biologic differences between papillary urothelial neoplasms of low malignant potential (PUNLMP) and low-grade (grade 1) papillary carcinomas of the bladder. METHODS: The rate of 2-year recurrence, the index of cell proliferation (Ki67), oncogene/antioncogene (c-erbB-2, p53) of 34 cases of PUNLMP and 46 cases of low-grade papillary carcinomas were analysed.RESULTS: The statistical significance of positive expression of Ki67 (P=0.001), p53(P=0.003) and rate of recurrence (P=0.020) was found between PUNLMP and low-grade papillary carcinomas, but no significance of positive expression of c-erbB-2 was found. CONCLUSION: The differences of biological behavior and clinical prognosis between PUNLMP and low-grade papillary carcinomas are obvious, but a long-term follow-up is advisable for the patients with PUNLMP.
