超声弹性成像与常规超声诊断甲状腺良恶性结节的对照研究

摘    要:目的探讨超声弹性成像(ultrasonic elastography,UE)技术与常规超声诊断甲状腺良恶性结节的价值。方法选择我院住院患者41例、58个甲状腺结节,术前均分别进行常规超声检查及实时超声弹性成像检查,术后不同超声类型检查结果相互比较,最后与病理结果对照分析。结果58个甲状腺结节中,良性43个,恶性15个。常规超声诊断甲状腺恶性结节的敏感性73.33%、特异性74.42%、准确性74.14%;弹性成像技术诊断甲状腺恶性结节的敏感性80%、特异性74.42%、准确性75.86%;常规超声和弹性成像技术联合诊断甲状腺恶性结节的敏感性93.33%、特异性88.37%、准确性89.66%。常规超声诊断准确度与弹性成像技术诊断准确度比较,无统计学意义,常规超声诊断准确度、弹性成像技术诊断准确度分别与联合技术诊断准确度相比,均有统计学意义。结论常规超声检查与弹性成像技术联合运用.可以大大提高诊断甲状腺良恶性结节的准确性。

Abstract:Objective To study the value of ultrasonic elastography (UE)and conventional ultrasound in differentiating benign from malignant thyroid nodules. Methods Forty-one patients with 58 thyroid lesions were examined by ultrasonic elastography and conventional ultrasound. All the cases were proved by surgical pathology. Results There were 43 benign and 15 malignant lesions. The sensitivity, specificity, and the accuracy of conventional ultrasound in the diagnosis of thyroid nodules were 73.33% ,74.42% and 74.14% respectively,and those of UE were 80% ,74.42% and 75.86%, while those of conventional ultrasound in conjunction with UE were 93.33% ,88.37% and 89.66%, Statistical results showed there was no significant difference in accuracy between conventional ultrasound and UE. There was significant difference in accuracy between conventional ultrasound and conventional ultrasound in conjunction with UE, and there was significant difference in accuracy between UE and conventional ultrasound in conjunction with UE. Conclusion The combination of conventional ultrasound and UE can improve the accuracy of differential diagnosis.
