妊娠期肝内胆汁淤积症胎盘组织中Caspase-3和Livin表达的相关性研究

摘    要:目的探讨妊娠期肝内胆汁淤积症(ICP)胎盘中凋亡调节基因Caspase-3和Livin表达的关系。方法以30例ICP患者(ICP组)及21例正常产妇(对照组)为研究对象,应用免疫组化方法测定胎盘凋亡调节基因Caspase-3和Livin的表达水平。结果①ICP组胎盘组织中细胞滋养细胞、合体滋养细胞、蜕膜细胞及间质细胞Caspase-3表达分别为(32.86±6.02)%、(21.57±7.52)%、(27.29±4.89)%、(18.14±3.58)%,明显高于对照组的(14.58±3.41)%、(9.58±2.26)%、(15.00±5.39)%、(9.17±2.20)%。两组比较,差异有显著性(P〈0.05)。②ICP组胎盘组织中细胞滋养细胞、合体滋养细胞、蜕膜细胞及间质细胞中Livin的表达分别为(7.43±2.17)%、(4.71±1.81)%、(4.57±1.21)%、(6.71±2.73)%,明显低于对照组的(14.38±3.24)%、(10.00±3.66)%、(8.75±2.97)%、(12.29±3.59)%。两组比较,差异有显著性(P〈0.05)。③ICP组胎盘组织中合体滋养细胞、细胞滋养细胞、蜕膜细胞及间质细胞Caspase-3和Livin的表达均呈负相关性,相关系数分别为:-0.61、-0.58、-0.57、-0.45,P值均小于0.05。结论ICP组胎盘组织中舍体滋养细胞、细胞滋养细胞、蜕膜细胞及间质细胞Caspase-3表达增高,Livin的表达降低,各细胞中Caspase-3和Livin的表达呈负相关性。ICP组胎盘功能减退与细胞凋亡密切相关,Caspase-3和Livin基因可能是ICP胎盘细胞凋亡的重要调控基因之一。

Abstract:Objective To investigate the relationship between the expressions of caspase - 3 and livin in placenta of patients with intrahepatic cholestasis of pregnancy (ICP). Methods Samples were obtained from 30 cases of ICP ( ICP group) and 21 normal pregnant women ( control group). The expressions of caspase -3 and livin in placenta tissues were detected by immunohistochemistry method. Results ①The expressions of caspase 3 in placental cytotrophoblast, syncytiotrophoblast, decidual cells and interstitial cells in ICP group were 32.86 ± 6.02 %, 21. 57 ±7.52%, 27.29 ±4.89% and 18.14 ±3.58%, respectively, which were significantly higher than those ( 14.58 ±3.41%, 9.58 ±2.26%, 15. 00 ± 5.39% and 9.17 ± 2.20% ) in control group ( P 〈 0.05). ②The expressions of livin were 7.43 ± 2.17%, 4.71 ± 1.81%, 4.57 ± 1. 21% and 6.71 ± 2.73%, respectively, which were significantly lower than those ( 14.38 ± 3.24%, 10. 00 ± 3.66%, 8.75 ± 2.97% and 12.29 ± 3.59 % ) in control group ( P 〈 0.05 ). ③There was a significantly negative correlation between the expressions of caspase - 3 and livin in placental syncytitrophoblast, cytotrophoblast, decidual cells and interstitial cells in ICP group, ( r = -0.61, -0.58, -0.57 and -0.45 respectively, P 〈 0.05 ). Conclusion ① The expression of caspase - 3 is high, while the expression of livin is low; and there is a negative eorrelation between the expressions of easpase - 3 and livin in placental syncytitrophoblast, cytotrophoblast, decidual cells and interstitial cells in ICP placenta. ②The dysfunction of placenta may closely relate to the apoptosis, and the regulation of caspase - 3 and livin expressions in placenta of patients with ICP may be one of the main reasons for placental apoptosis.
