眼睑松弛症的手术治疗分析

目的:探讨眼睑松弛症的手术治疗方法及效果。方法:对15例26眼眼睑松弛症患者均在停止发作并静止6mo以上手术。对于上睑皮肤松弛、上睑下垂、泪腺脱垂和睑裂横径缩短均采用重睑切口,上睑下垂视提上睑肌腱膜有无断裂采取提上睑肌腱膜修复或折叠术。泪腺脱垂全层缝合泪腺组织固定于眶泪腺窝处的骨膜上。睑裂横径缩短将外眦韧带缝合固定于外侧眶缘骨膜,下睑内翻做平行于睑缘的皮肤切口,去除多余皮肤及部分眼轮匝肌。结果:15例患者皮肤松弛得到明显改善,其中上睑下垂、睑裂横径缩短和下睑内翻,也均得到满意矫正,泪腺脱垂1例明显改善,余复位良好。随访6mo～2a,无复发。结论:手术治疗安全,效果确切。

Analysis of the operation for dermatolysis palpebrarum

AIM:To find the best operation method for dermatolysis palpebrarum.·METHODS: We operated on 15 cases (26 eyes) with dermatolysis palpebrarum which stopped attack and was static for more than six months.We use the incision which using in operation for double eyelid for upper eyelid cutis laxa,dislocation of lacrimal gland and shortening of fissura palpebrae trans diameter. And the levator muscle of upper eyelid aponeurosis prothesis or Plication was choosed when the levator muscle of upper eyelid aponeurosis was broken or not. In the operation of dislocation of lacrimal gland we sewed up lacrimal gland full-thickness and fixed it to the periosteum of lacrimal fossa. And for the shorted transverse diamaeter of coloboma of eyelid we sewed up the external palpebral ligament and fixed it to the periosteum of lateral orbital margin. And when followed with lower eyelid entropion we set a incision which paralleled with limbal palpebralis as well as removaled the unnecessary skin and sphincter palpebrarum. ·RESULTS: To all the 15 cases,the cutis laxa had been amended obviously;lapsus palpebrae superioris,shorted transverse diamaeter of coloboma of eyelid and lower eyelid entropion had been rectified satisfactorily;and dislocation of lacrimal gland had been amended in one case,and others have been reposited well.All these cases we followed up for six monthes to two years occurred no recurrence.·CONCLUSION: The operation seems to be saft and the result may be sure.
