经鼻给予布托啡诺对丙泊酚注射痛的影响

目的：比较经鼻给予不同剂量布托啡诺对丙泊酚注射痛的影响。方法：采用随机、双盲、对照研究。将ASAⅠ～Ⅱ级择期行开颅手术的90例患者随机分为三组,每组各30例。所有患者麻醉诱导前15 min经鼻给药。Ⅰ组给予生理盐水0.3 ml,Ⅱ组给予布托啡诺2 mg,Ⅲ组给予布托啡诺3 mg。以5 mg/s速度给予患者1/4总剂量的丙泊酚（2 mg/kg）。静脉注射丙泊酚时采用4点法对疼痛评分,0=无痛,1=轻度疼痛,2=中度疼痛,3=严重疼痛。结果：Ⅰ组有70%患者诉疼痛;Ⅱ组和Ⅲ组分别为53.3%和26.7%。Ⅲ组跟Ⅰ组比较,疼痛发生率明显降低（P〈0.05）。结论：麻醉前15 min经鼻给予布托啡诺3 mg能预防丙泊酚注射痛。

The effect of intranasal Butorphanol on the pain during injection of Propofol

Objective： To compare the efficacy of intranasal Butorphanol with different doses for prevention of Propofol-induced pain.Methods： 90 patients of ASAⅠ-Ⅱ undergoing selective craniotomy were divided into three groups,group Ⅰ received saline 0.3 ml,group Ⅱreceived Butorphanol 2 mg,and group Ⅲ received Butorphanol 3 mg.The intranasal medicine was giving 15 minutes before anesthesia induction.All patients received one-fourth of the total dose of Propofol（2 mg/kg） in the speed of 5 mg/s.Assessment of pain with Propofol was done by using a four-point scale,0=no pain,1=mild pain,2=moderate pain and 3=severe pain at the time of Propofol injection.Results： In group Ⅰ,70% patients had pain during Propofol injection,and in group Ⅱand group Ⅲ were 53.3% and 26.7%,respectively.The patients with injection pain in group Ⅲ were significantly less than group Ⅰ（P0.05）.Conclusion： The application of intranasal Butorphanol 3 mg 15 min before anesthesia can effectively attenuate the pain associated with Propofol.
