胶囊内镜与单气囊小肠镜对可疑小肠疾病的诊断价值

目的比较胶囊内镜与单气囊小肠镜对小肠疾病的诊断价值。方法 2009年6月-2011年12月共有37例疑诊为小肠疾病的患者接受了胶囊内镜(CE)和单气囊小肠镜(SBE)检查。比较两种检查方法对小肠疾病的检出情况。结果在37例患者中,CE检查发现阳性病变28例,总体诊断率为75.68%(28/37),SBE检查发现阳性病变33例,总体诊断率为89.19%(33/37),两者之间差异无统计学意义(P=0.221)。CE和SBE检查结果不一致,CE检查阳性而SBE检查阴性3例,CE检查阴性而SBE检查阳性8例,CE和SBE检查均阴性1例。CE全小肠检查完成率为94.59%(35/37);SBE全小肠检查完成率为57.14%(4/7)。两种检查方法均未发生严重并发症。结论胶囊内镜和单气囊小肠镜对小肠疾病的诊断价值相当。

Diagnostic value of capsule endoscopy and single balloon endoscopy for patients with suspected small bowel disease

Objective To compare the diagnostic value of capsule endoscopy(CE) and single balloon endoscopy(SBE) for patients with supected small bowel disease.Methods From Jun.2009 to Dec.2011,37 patients with suspected small bowel disease were enrolled in the present investigation.CE was performed before SBE.The detection rate,diagnostic accuracy,tolerance and frequency of adverse events of these two modalities were analyzed.Results Small bowel lesions showed different results by CE and SBE.The overall detection rate of small bowel diseases by CE(75.68%,28/37) was higher than that by SBE(89.19%,33/37),but the difference was not significant(P= 0.221).Lesions were detected by SBE in 8 out of 9 patients in whom CE had a negative result.Both CE and SBE showed negative result in one patient.On the other hand,small bowel lesions were identified by CE all 3 patients after negative evaluations by SBE.Total enteroscopy was achieved by CE in 35 of the 37 patients(94.59%).Total enteroscopy was achieved by SBE in 4 of 7 patients(57.14 %).There were no severe complications during or after either of the two procedures.Conclusion CE and SBE are nearly equal in their ability to detect small bowel lesions.
