冠状动脉侧支循环对冠心病患者预后的影响

目的 :探讨冠状动脉侧支循环对冠心病患者预后的影响。方法 :对选择性冠状动脉造影证实主要分支狭窄≥75 %的 42例冠心病患者 ,依据冠状动脉造影影像将研究对象分为冠心病伴或不伴侧支循环两组。统计分析两组不稳定型心绞痛、心肌梗死、室壁瘤的发生率 ;心脏彩色多普勒检测左室射血分数 ,1 2导同步心电图测定QTc离散度。结果 :伴与不伴冠状动脉侧支循环两组不稳定型心绞痛、心肌梗死的发生率差异不显著 (P >0 .0 5) ;但无侧支循环患者乳头肌功能失调、室壁瘤的发生率较高 (P <0 .0 1 ) ,左室射血分数低 ,QTc离散度增加。结论 :侧支循环的建立可改善冠心病患者预后

THE EFFECTS OF CORONARY COLLATERAL CIRCULATION ON PROGNOSIS OF CORONARY HEART DISEASE

Objective:To explore the effects of coronary collateral circulation on prognosis of coronary heart disease.Methods:Thirty nine patients with coronary heart disease were divided into two groups:Coronary heart disease without coronary collateral circulation and coronary heart disease with coronary collateral circulation according to coronary angiography.To analyze the clinical data of two groups including incidence of unstable prevalence of unstable angina,myocardial infarction,aneurysm ventricular,left ventricular ejection fraction and QTc dispersion.Results:The incidence of unstable angina and myocardial infarction were no significant difference between coronary heart disease with coronary collateral circulation patients and coronary heart disease without coronary collateral circulation patients.Patients of noncoronary collateral circulation had high incidence of dysfunction of papillary muscle and aneurysm ventricular,left ventricular ejection was decreased and QTc dispersion was increased.Conclusions:Our studies demonstrated that prognosis of coronary heart disease had closed to the formation of coronary collateral circulation.
