环磷酰胺冲击治疗对系统性红斑狼疮并发肺动脉高压的作用

摘    要:目的研究环磷酰胺冲击治疗对系统性红斑狼疮并发肺动脉高压的有效性和安全性。方法将符合入选条件的住院患者33例随机分为两组,试验组18例用环磷酰胺每月1次静脉冲击治疗,对照组15例给予依那普利,疗程均为12周。结果疗程结束后两组患者肺动脉收缩压显著降低,6min步行距离明显改善;试验组在改善临床指标及生活质量方面均优于对照组;试验组少数患者发生不良反应,经对症处理后均能缓解,有1例患者因发生肺部感染而退出试验。结论每月1次静脉注射环磷酰胺可有效降低系统性红斑狼疮并发的肺动脉高压,改善患者的生活质量。

Abstract:Objective To study the effects and safety of cyclophosphamide on the treatments of pulmonary hypertension associated with systemic lupus erythematosus. Methods 33 qualified inpatients were randomly divided into the trial group (18 cases, given cyclophosphamide pulse therapy monthly) and the control group (15 cases, given enalapril).The clinical indexes were observed and compared after all the patients underwent one treatment course of 12 weeks. Results After 12-week treatment the two therapies all showed effective. The pulmonary artery systolic pressure decreased and the 6 min walk distance increased. The improvements of laboratory indexes and the quality of life of the trial group were obviously greater than those of the control group. Compared with the control group, there were some side effects in the trial group, which could be extenuates by symptomatic therapy. One patient withdrew from the trial because of pulmonary infection needing admission. Conclusion Cyclophosphamide pulse therapy monthly can effectively relieve pulmonary hypertension associated with systemic lupus erythematosus and obviously improve patients' quality of life.
