老年人肠梗阻234例分析

目的总结急性肠梗阻临床诊治经验。方法回顾性分析1996年3月至2003年3月收治的234例老年急性肠梗阻临床资料。结果234例急性肠梗阻中165例有一种或一种以上合并症,机械性肠梗阻为215例,占91.9%,其中腹腔内肿瘤引起的98例,其余为粪块阻塞、腹股沟疝嵌顿、乙状结肠扭转、柿石梗阻、胆石梗阻等,手术治疗105例治愈、好转102例,死亡3例;非手术治疗129例,治愈、好转127例,死亡2例,死亡原因为脏器功能衰竭。结论(1)高龄肠梗阻的病因中肿瘤占首位,肠粘连其次。(2)肿瘤引起的高龄肠梗阻如无手术禁忌证,应争取手术治疗。

Analysis of 234 cases of elderly patients with acute intestinal obstraction

Objective To assess the outcomes of treatment in elderly patients with acute intestinal obstruction.Methods Clinical data of 234 elderly patients with acute intestinal obstruction from 1996.3 to 2003.3 were retrospectively analyzed.Results 165 cases had one coexistent disease or more,215 cases had mechanical intestinal obstruction,which accounted for 91.9%.Of 215 patients with mechanical intestinal obstruction,abdominal mass was 98 cases,others were feculent mass obstruction,incarcerated hernia of the groin,sigmoid colon volvulus,persimmon stone obstruction,and gallstone obstruction.205 cases were cured and 20 cases were improved,5 cases died,the cause of death was dysfunctions of organ,4 cases left untreated.Conclusions Tumour is main cause of intestinal obstruction in elderly patients.If there were no contraindication,operation should be done in the patients with tumour intestinal obstruction.
