腹腔镜下行直肠癌前切除术23例临床观察

目的探讨腹腔镜下行直肠癌前切除术（LAR）的临床效果。方法回顾性分析23例腹腔镜下LAR的临床资料，与同期传统手术方法行直肠癌前切除术25例进行对比。结果本组均在全麻下完成腹腔镜下LAR．并且达到TME标准，术后患者肛门功能不同程度地得到保留，随访1年未发现明显转移癌灶及局部复发灶。腹壁切口长度、肿瘤下缘距肛缘的长度、术中失血量、术后腹腔引流量、住院时间明显低于传统手术组，但手术时间明显延长。结论腹腔镜下行LAR有明显优势，手术创伤小、术中出血少、术后恢复快、保肛率增加，对于部分超低位直肠癌行前切除术，仍可不同程度地保留肛门功能，同样可达到直肠癌根治要求及TME标准，对提高患者的生活质量具有重要意义。

Clinical observation of the laparoscopic rectal resection:23 cases

Objective To study the clinical results of laparoscopic anterior resection of rectal cancer. Methods Forty- eight cases were enrolled in this study, 23 cases undergoing laparescopie anterior resection, and 25 cases undergoing open radical resection. Results Twenty-three patients underwent laparoscopic anterior resection of rectal cancer through general anesthesia, and attained the standard of the straight mesenteric resection TME. Postoperative patient s＇s anal function re- served to different extent. Patients were followed up for one year and no tumor recurrence or metastasis was found.Abdomi- nal incision length, tumor from the length of the anus inferior margin, intra-operative blood loss, postoperative celiac lead flow, and the hospital stay in laparoscopic group were significantly better than the traditional group, but operation time was significantly longer. Conclusion Compared with the traditional opention, enormous advantages were gained in laparoscopic anterior resection of rectal cancer, such as less bleeding and better recovery. And the anus rate increases and anal func- tion can be reserved to some degrees for some pationts with lower colorectal cancer. Bob requirement and standard for vesssed colorectal cancer can be achieved in laparoscopic anterior resection of rectal cancer and for patients,laparoscopic surgery way means a lot for improvement of the quility of life.
