布托啡诺联合吗啡用于剖宫产术后镇痛的临床观察

目的探讨布托啡诺联合吗啡在剖宫产术后硬膜外镇痛中的临床效果。方法将2008年5月-2012年1月我院收治的200例剖宫产产妇随机均分为吗啡组（对照组）和布托啡诺联合吗啡组（观察组），每组100例。两组产妇均行腰硬联合阻滞麻醉，观察术后2h、4h、8h、12h、24h，48h的视觉模拟评分尺（VAS）评分、Ramesay评分及不良反应。结果两组产妇的镇痛效果比较，在第2、4小时的位置无统计学意义（P〉0．05），在8h、12h、24h、36h、48h的镇痛效果均明显好于对照组，差异有统计学意义（P〈0．05）。两组产妇的镇静效果评分比较，无明显统计学意义（P〉0．05），两组的恶心、呕吐、皮肤瘙痒和呼吸抑制的不良反应比较差异有统计学意义（P〈0．05）。结论布托啡诺联合吗啡在剖宫产术后硬膜外镇痛能有效地提高术后镇痛质量，是一种安全、简便、有效的产后镇痛方法。

The clinical observation of butorphanol combined with morphine for postoperative analgesia after cesarean section

Objective To investigate the clinical effect of butorphanol combined with morphine for cesarean section postop-erative epidural analgesia.Methods A total 200 of cases of cesarean section treated in our hospital from may 2008 to Jan-uary 2012 were randomly devided into two groups: the morphine group(control group) and Butorphanol combined with morphine group(observation group),100 cases in each group.Two groups of women underwent lumbar epidural combined anesthesia,was observed after 2 hours,4 hours,8 hours,12 hours,24 hours,48 hours of visual analogue scale(VAS) score,Ramesay score and adverse reaction.Results The results of two groups of maternal analgesia effect comparison,in 2 hours,4 hours location was not statistically significant(P > 0.05);8 hours,12 hours,24 hours,36 hours,48 hours of analgesia were significantly better in the observation group,and had the statistical significance(P < 0.05).Two groups of maternal sedation score comparison had no significant difference(P > 0.05);two groups of nausea and vomiting skin itching and respiratory depression adverse reactions had significant difference(P < 0.05).Conclusion Butorphanol combined with morphine for cesarean section postoperative epidural analgesia can effectively improve the quality of postoperative analgesi-a,is a safe,simple,effective method for postpartum analgesia.
