摘    要:目的探讨动机性访谈对老年2型糖尿病患者行为改变和糖化血红蛋白的影响。方法将80例住院老年2型糖尿病患者随机分为观察组和对照组各40例。观察组采用动机性访谈干预;对照组采用常规健康教育方法。干预前及干预后6周、3个月、6个月分别评估两组患者自我管理能力,同时在干预前、干预后第3、6个月检测糖化血红蛋白。结果观察组干预后不同时间点糖尿病自我管理能力总分及多维度均分显著高于干预前及对照组(P<0.05,P<0.01);与干预前比较,观察组患者干预后第3个月HbAlc显著下降(P<0.05)。结论动机性访谈在提高患者糖尿病自我管理能力及血糖控制水平方面具有明显的促进作用。

Abstract:Objective To evaluate impact of motivational interviewing on behavioral change and levels of glycosylated hemoglobin(HbA1c) in elderly patients with type 2 diabetes mellitus(T2DM). Methods Eighty elderly T2DM patients were randomly divided into an observation group and a control group of 40 receiving either diabetic education with motivational interviewing technique or conventional health education.Self-management ability of the two groups were evaluated before and 6 weeks,3 months,6 months after the intervention;HbA1c was tested before and 3 months,6 months after the intervention. Results Significant time effect and group effect were found in total score and multiple dimension scores of diabetes self-management ability(P<0.05,P<0.01);the value of HbA1c was decreased 3 months after the intervention in the observation group(P<0.05). Conclusion Motivational interviewing applied to diabetic education can promote diabetes self-management ability of patients and maintain better glycemic control.
