桡骨远端锁定钛板内固定加中医药治疗锁骨粉碎性骨折的疗效

目的：探讨锁骨粉碎性骨折采用桡骨远端锁定钛板内固定加中医药治疗临床分析。方法：本次研究对象共15例，选自我院骨科2010年1月-2012年1月收治的锁骨粉碎性骨折的患者，采取桡骨远端锁定钛板内固定治疗，回顾性分析其临床资料。患者在手术后均进行中医药辅助治疗。结果：本纽15例患者，术中为45-190ml出血量，手术时间为32～115min。平均随访10个月，骨折复位X线检查均呈良好表现，获得了可靠的内固定。骨折在随访期间均良好愈合，无皮肤坏死感染、畸形愈合、螺钉拔出、延迟愈合、钛板断裂等并发症。Rockwood法评估肩关节功能。优10例，占66．7％；良4例，占26．7％；差1例，占6．7％，总有效率为93．3％。胸锁关节处在肩关节活动时有隐痛发生2例。合并胸锁关节脱位的患者中，稍有前凸畸形在骨折愈合后发生。结论：锁骨粉碎性骨折采用桡骨远端锁定钛板内固定加中医药辅助治疗，具有一定的安全性和有效性，显著改善了肩关节功能，使患者的生存质量得到了最大程度的保障。应用中医药辅助治疗，能促进患者的康复过程，对患者预后起到较为满意的效果。

The effect of treating comminuted clavicular fracture by fixation of distal radius locking titanium plate plus TCM

Objective:To investigate the clinical analysis of treating comminuted clavicular fracture by fixation of distal radius locking titanium plate plus TCM.Methods:15 cases of comminuted clavicular fracture selected from hospital orthopedic from January 2010 to January 2012,to take fixation of distal radius locking titanium plate method,and to analysis the clinical data.TCM for patients after surgery.Results:The intraoperative bleeding for 45～190ml,and operation time for 32～115min.followed up 10 months,the fractures reset X-ray examination showed a good performance,a reliable internal fixation.Fracture during follow-up are good healing,skin necrosis,infection,malunion,screw pull-out,delayed union,titanium plate fracture and other complications.Using Rockwood method to assess the shoulder function.Excellent in 10 cases(66.7%);good in 4 cases,26.7%;poor in 1 case(6.7%),total effective rate was 93.3%.Sternoclavicular joint in shoulder range of motion when the pain occurred 2 cases.A slight lordosis deformity of patients that with sternoclavicular joint dislocation occurred after fracture healing.Conclusion:The method of treating comminuted clavicular fracture by fixation of distal radius locking titanium plate plus TCM is safety and efficacy,that can significant improve shoulder and the quality of life.TCM can promote the rehabilitation process of patients,has a satisfactory results.
