机械通气人机对抗276例的临床分析与护理

[目的]探讨在机械通气时保持人机协调有利于提早撤机和疾病康复。[方法]回顾性分析2007年1月—2011年12月华中科技大学同济医学院附属同济医院重症医学科收治的建立人工气道进行机械通气病人中,276例发生人机对抗的原因。[结果]及时发现和评估人机对抗表现,强化呼吸道管理,采取适当的措施保持人机协调,取得了满意效果。[结论]气道阻塞是人机对抗的主要原因,设置合适的通气模式和参数,祛除病因,解除气道痉挛,联用镇静、肌松、镇痛治疗,加强护理,可改善病人通气状况,人机对抗得以纠正。

Clinical analysis and nursing care of 276 cases of the machinery ventilation Patient-ventilator asynchrony

Objective:To probe into mechanical ventilation,to keep man-machine coordination,in favour of disease recovery.Methods:Retrospective analysis of 276 cases of Patient-ventilator asynchrony from January 2007 to December 2011 in division of critical care of affiliated tongji hospital of tongji medical college of huazhong scientific and technical University.Results:To discovering and evaluate the performance of Patient-ventilator asynchrony,strengthen the respiratory tract,and to take appropriate measures to maintain the human-machine coordination achieved satisfactory results.Conclusion:Airway obstruction is the main reason for Patient-ventilator asynchrony.To set the appropriate ventilation mode and parameters,to eliminate the cause,to relieve airway spasm,combined with sedation,muscle relaxation,pain treatment,intensive care can improve patients’ventilation conditions,and Patient-ventilator asynchrony will be corrected.
