心包腔积液在慢性充血性心力衰竭诊疗中的意义

目的 :研究慢性充血性心力衰竭 (CHF)病人心包腔积液 (PE)的检出率及其在CHF发病中的临床意义。方法 :对 184例CHF病人临床资料与超声心动图 ,X线胸片等发现的PE进行相关性研究 ,并对PE与胸腔积液 (TE)间的相关性进行探讨。结果 :全组病人PE检出率 36 .9% ,其中扩心病 5 1.6 % ,风心病 30 %。X线胸片TE检出率 5 0 % ,风心病TE发生率 70 %。X线胸片和 /或胸部B超证实有TE并不能预测PE存在。有PE病人射血分数 (EF)为 (31.6± 13.9) % ,无PE者为 (42 .4 4± 17.9) % (P <0 .0 1)。合并TE者EF为 (36 .7± 16 .5 ) % ,无TE者为 (42 .5± 16 .3) % (P<0 .0 5 )。结论 :CHF病人PE和TE的发生率高 ,PE与TE的相关性差 ,心功能状态与TE和PE的发生呈明显的负相关关系

Clinical Significance of Pericardial Effusion in Patients With Chronic Heart Failure

Objective: Our aim was to study the incidence of pericardial effusion(PE) in patients with chronic heart failure(CHF) and its clinical significance. Methods: We retrospectively studied the correlation between the clinical data and incidence of PE found with echocardiography and X-ray in 184 patients with CHF. We also studied the relation between PE and thoracic effusion(TE). Results: The incidence of PE and TE were 36.9% and 50%. Incidence of PE were 51.6% and 30% in dilated cardiomyopathy and rheumatic heart disease respectively. No correlation existed between PE and TE (P>0.05), but PE and TE were negatively correlated with ejection fraction(EF) (P<0.05). Conclusion: The incidence of PE and TE in CHF are relatively high. Incidence of PE and TE are negatively correlated with cardiac function, but the correlation between PE and TE in CHF is low.
