Die Zeitwahl der Implantatentnahme nach Osteosynthese

Abstract:Zusammenfassung Infolge einer veränderten Knochenheilung nach neuartigen Osteosyntheseverfahren unterliegt die Indikationsstellung zur Implantatentfernung strengen Kriterien, die Erfahrung und Schulung voraussetzen. Im Gegensatz zur konservativen Behandlung ist nach Osteosynthese die Callusbildung umgekehrt proportional dem erreichten Stabilisierungsgrad. Das bedeutet, daß der durch das operative Verfahren erlangte Stabilisierungsgrad, die Knochenheilung und das subjektive Befinden des Patienten genauestens überprüft werden müssen. Nach komplikationsloser Knochenheilung können entsprechend den Erfahrungswerten die Implantatentnahmen vorgenommen werden. In jedem Falle sollten folgende Mindestbelassungszeiten in Monaten für die Implantate beachtet werden:
Removal of implants and its period after osteosynthesis
Summary In consequence of an altered healing-process of bone after new osteosynthesis-treatment, the indications for the removal of implants must reconcile with strict criteria, which presumes experience and practice. Contrary to conservative treatment, the callus after osteosynthesis is inversely proportional to the obtained stabilisation. That means, that one must scrutinize the point of stabilisation obtained by operative treatment, the healing-process of the bone, and the subjective condition of the patient. After healing without any complications, the implants can be taken off according to experience. If there is any sign of delayed ossification, however, one must decide individually in every case.The same criteria, which are valid for primary osteosynthesis, are valid both for corrective operations and pseudarthrosis, too. In the event of an osteomyelitis, the material is always taken off, when its task, the stabilisation, is no longer accomplished, or when the ossification is satisfactory. After ossification the implant should be taken off in any case, except there exist contraindications such as narcotic problems, old age and other diseases, which are well to the fore.From the above mentions results, that there are no common rules for taking off an implant; moreover, in the question of removing the implant, there is decisive exclusively the degree of ossification such as visible in the roentgenogram.
