直肠癌180例临床分析

目的：探讨直肠癌的临床特征及直肠癌手术方式的选择。方法：回顾分析行外科手术治疗的180例患者直肠癌的病例资料。分析不同分期与不同手术方式的治疗效果。结果：直肠癌中青年患者较多，多位于直肠中下段，Dukes分期以B期，C期居多，5年生存率DukeA,B,C期分别为98．0％，56．0％，22．0％，中下段癌保肛手术，Miles术后局部复发率分别为6．3％，21．5％，结论中下段直肠癌保肛手术疗效肯定，在高危人群中筛选早期癌患者和严格按照全直肠系膜切除标准对进展期直肠癌进行手术，是提高直肠癌远期疗效的关键。

Rectal Cancer A Clinical Analysis of 180 Cases

Objective To summary the clinical feature and discuss the operation model of rectal cnacer.Methods The treatment and follow-up were retrospectively analysed on 180 cases of rectal cancer hospitalized from Jan 1995 to Jan 1997.Results The most of patients with rectal cancer were young and middle-aged,the cancers located in middle and inferior segment.The B and C stage hold most according to Dukes classification.The 5-year survival rate in Duke A?B?C was 98.0%,56.0%,22.0% respectively,the local recurrence rate of anus saving resection and Miles operation hold 6.3%?21.5% respectively.Conclusion The effect of anus saving resection is positive in middle and inferior segment of rectal cancer,screening early rectal cancer patients in high-risk group and performing operation according to the standard of TME strictly in advanced rectal cancer are an effective method to improve the long-term effect.
