8例肝脏炎性假瘤的超声造影表现分析

目的探讨肝脏炎性假瘤（Inflammatory pseudotumor of liver，IPL）的超声造影（Contrast enhanced ultrasound，CEUS）表现，以提高对该病的认识。方法2004年1月-2009年12月，对在我院行常规超声及CEUS检查并经手术或超声引导下活检证实为IPL的8例患者进行回顾性分析。结果3例IPL表现为造影三相呈无增强，5例有增强，其中3例早期动脉相呈高增强，2例为等增强；4例在门脉相延迟相呈低增强，1例在门脉相呈等增强，延迟相病灶局部呈低增强。结论IPL的超声造影表现存在多样性，对于超声造影疑为恶性病灶而临床表现不支持时，不能完全除外IPL的可能，有必要行超声引导下活检以明确诊断。

Analysis of manifestation of contrast enhanced ultrasound in 8 cases with inflammatory pseudotumor of the liver

Objective To investigate the features of inflammatory pseudotumor of the liver （IPL） on contrast enhanced ultrasound （CEUS） for elevating regeonition of this disease. Methods From January 2004 to December 2009, 8 cases with conventional uhrasonography and contrast enhanced ultrasound and via operation or ultrasound guided biopsy confirmed diagnosis as IPL were analyzed retrospectively. Results Three cases of IPL showed as no enhancement in the three phase and 5 cases had enhancement.Among them,3 cases showed hyperenhaneement and 2 cases showed isoenhancement in the arterial phase. Four cases showed hypoenhancement in the portal venous phase and in the delayed phase.One ease showed isoenhanecment in the portal venous phase and local lesion showed hypoenhaneement in the delayed phase. Conclusion The IPL manifestes diversely on CEUS. If the lesion is suspected to be malignant and the clinical manifestations doesn＇t support it, ultrasound guided biopsy should be performed to confirm the diagnosis.
