对肝脏转移性肿瘤CT表现的分析及诊断

目的:分析CT扫描对肝脏转移性肿瘤的诊断价值,提高CT对诊断肝脏转移性肿瘤重要性的认识。方法:对32例肝脏转移性肿瘤经手术和(或)病理证实,其原发灶有肺癌10例,胃癌9例,乳腺癌4例,胰腺癌3例,结肠癌2例,食管癌1例、胆总管癌1例,子宫内膜癌1例,以及肝恶性黑色素瘤1例。全部病例均经CT平扫和增强扫描。17例还于增强后1min、3min、5min做了延迟扫描。结果:CT平扫准确证实了多数病例(28/32)表现为肝内多发性病灶且大多病灶(22/28)能被清楚辨认。增强扫描后可见如下CT表现:病灶整体轻度强化;病灶边缘轻度强化;病灶基本不强化;病灶缩小乃至消失;出现平扫时未检出的病灶。延迟扫描后,未见比增强扫描时更有意义的CT表现。结论:CT平扫和增强扫描对肝脏转移性肿瘤的诊断具有重要实用价值。

Analysis of CT Findings and The Diagnosisin Metastatic Hepatic Carcinoma

Objective To analyze the diagnstic value of CT scan in metastatic hepaticcarcinoma,Increase CT to examinemetastatic hepatic carcinoma the cognition ofimportance. Methods Metastatic hepatic carcinoma in 32 cases whose primary niduses originated from 10 cases of lung cancer,9 gastric carcinoma,4 breast,3pancreatic,2 colic,1 esophageal carcinoma,1 manager cancer,1 endometr ialadenocarcinoma and Malignant melanin in liver lump were proven by the operations and(or) the pathologic diagnoses.All patients underwent CT plain scan and enhancementscan.17 patients underwent additional Ctdelayed scan at 1,3,and 5 min after enhancing.Results CT plain scan accurately demonstrated that most patients(28/32) were found to be multiple niduses and most niduses of them(22/28) could be clearly recognized.Following enhancement scan CT findings of the niduses were found as below:complete nidus was slightly enhancing;rim enhancing;nidus itselfnon-enhancing;nidus itself reduced or even lost;some niduses Which could not found during plain scan were displayed out.Delayed scan no more interesting CT findings found than enhancement scan.Conclusion CT plain scan and enhancement scan have significant applicability in the diagnosis of metastatic hepatic carcinoma .
