二期手术切除为主综合治疗中晚期肝癌36例体会

目的　探讨二期手术切除为主综合治疗中晚期肝癌的效果。方法　回顾我院 1993年 4月～ 2 0 0 1年 3月收治的二期手术切除的中晚期肝癌患者 3 6例 ,术前均行经皮股肝动脉化疗栓塞 (TACE) ,2 3例术前行B超引导下无水酒精瘤体内注射 (PEI) ,本组 3 6例术中均行经胃十二指肠动脉置管至肝动脉 ,18例经胃网膜右静脉置管至门静脉 ,皮下埋植化疗药泵 ,术后区域化疗。结果　以寿命表法计算 1、2、3、4、5年累计生存率 ,分别为 83 3 3 %、69 44 %、65 5 5 %、5 5 5 5 %、47 2 2 % ,中位生存期 3 2 5个月。结论　积极的综合法疗可延长中晚期肝癌患者生存期 ,延迟复发 ,效果良好

Second Stage Resection Treatment of Patients with Advanced Primary Liver Carcinoma

Objective To investigate the effect of second stage resection treatment of patients with advanced primary liver carcinoma.Methods 36 patients with advanced primary liver carcinoma treated by second stage resection in our hospital from April 1993 to March 2001 were reviewed.All cases were performed transcatheter arterial chemoembolization(TACE)and 23 cases were performed percutaneous ethanol injection(PEI)guided by B ultrasound before the operation.All cases took local therapy by hepatic arterial infusion port,18 cases also by portal vein infusion port after the operation.Results The overall 1-,2-,3-,4-and 5-year survival rates of 36 patients were 83.33%,69.44%,65.55%,55.55% and 47.22% respectively.The median survival rate was 32.5 months.Conclusions Second stage resection treatment of patients with advanced primary liver carcinoma may prolong survival period and delay recurrence.
