十二指肠破裂治疗体会

目的：探讨十二指肠破裂一期吻合，经十二指肠留置空肠营养管以提供肠内营养、预防十二指肠瘘或促进瘘愈合的疗效。方法；2例经十二指肠球部置空肠营养管，一期吻合十二指肠破裂，加胃空肠吻合及空肠输入袢、输出袢侧侧吻合。结果：1例未并发瘘；1例并发瘘，经空肠营养管供给营养，2周后愈合。结论：留置空肠营养管能有效地预防十二指肠瘘，如已并发瘘，也可促进瘘的愈合。

Understanding in the treatment of 2 cases of duodenal rupture

Objective:To explore the earlly diagnosis and treatment of rupture of duodenum.Methods:2 Cases of duodenal rupture were retrospectively analyzed.Primary repair was performed in the two cases.Nutrient canal was placed into jejunum via.duodenal bulb,then gastrojujuno stomy and side anastomosis with afferent and efferent loop of jejunum.Results:One was cured with no fistula,another case was complicated,with fistula of duodenum after two weeks.Cured!with enteral nutrition,via jejunum nutrient canal.Conclusion:Jejunum nutrient canal can effectively prevent from fistula of duodenum.It is important for the patient complicated with fistula of duodenunm,it is also helps heal of fistula with enteral nutrition.
