浅层脂肪抽吸术后皮肤质量的改变

目的：探讨浅层脂肪抽吸术对抽吸区皮肤质量的影响.方法：对4头家猪进行浅层脂肪抽吸手术，分别于术后2周、4周、2个月、4个月收集术区以及对照侧全层皮肤标本，进行组织形态学观察，包括皮下脂肪厚度变化、组织学变化、真皮内的成纤维细胞的数量变化、真皮及皮下组织中的纤维变化进行了对照研究.结果：大体标本显示抽吸区皮下脂肪厚度降低，抽吸区的皮肤收缩;HE染色显示手术区与对照组脂肪细胞形态无改变，成纤维细胞数量增加，脂肪组织结构于术后4个月恢复正常;胶原VG染色显示术区胶原沉积且排列紊乱，网状纤维及弹力纤维于术后2个月显著增生，于4个月时基本恢复正常.结论：浅层脂肪抽吸术通过破坏真皮下脂肪，损伤、刺激真皮收缩而引起皮肤显著收缩;浅层脂肪抽吸术后皮下脂肪内抽吸隧道形成新的纤维网，皮肤重建在真皮层产生新的胶原，使皮肤收缩。

The basic study of superficial subdermal liposuction in the quality change of the overlying skin

Objective To investigate the pathology of the overlying skin in the treated area after superficial subdermal liposuction on animals.Methods Superficial subdermal liposuction were operated on four pigs(3 female pigs and 1 male pig).Tissue samples of whole layer skin were harvested from the overlying skin on 2,4,8 and 16 post-operation weeks for histomorphology examination including macroscopical observation and histological study and the quantity of fibroblast and fibro-change in dermal.Results Macroscopical observation showed the thickness of the subcutaneous adipose of the treated site is thinner than the controlled sites.The skin overlying the treated area contracted.Histological study(HE) showed no significant difference in adipocyte morphology between the two kind of samples.The amounts of the fibroblast in the dermal increased.The structure of fat tissue recovered normally during 4 months after SSL.Collagen VG staining showed the collagen fibers deposited and irregular.The elastic and reticular fibers proliferated significantly on 2 months after treatment and restored to normal on 4 months after SSL.Conclusion The significant contraction of the skin is secondary to dermal contracture caused by surgical "trauma" inflicted on the undersurface of the dermis.In addition superficial subdermal liposuction creates a new fibrous network underneath to support the skin,and skin resurfacing generates new collagen in the dermis.
