97例宫内节育器嵌顿因素临床分析

目的 :探讨宫内节育器 (IUD)嵌顿的原因。方法 :通过对 978行IUD取出术中发现的 97例IUD嵌顿因素作了回顾性调查 ,进行了统计学处理与临床分析。结果 :(1)IUD嵌顿率随年龄的增长而增高 (P <0 .0 5 ) ;(2 )人流术中安置者嵌顿率明显高于月经净后安置者 (P <0 .0 0 5 ) ;带器时间越长 ,嵌顿率越高 (P <0 .0 0 5 ) ;IUD的嵌顿率与IUD的种类也有明显关系 ,金属类环嵌顿率最高 (P <0 .0 0 5 )。结论 :对在人流术中安环、取环时年龄偏大、安环时间较长、安置金属类圆环要求取环的妇女 ,出现嵌顿环的几率偏大 ,在取环时应尽量在B超监测下取环 ,可以增加取环成功的机会。

Analysis of Factors of Intrauterine Device (IUD) Partly Trapped into the Wall of Uterine from 97 Cases

Objectives:To analyze factors attributing to intrauterine device (IUD) partly trapped into the wall of uterine. Methods:Retrospective study was performed on 97 cases with IUD partly trapped in the wall of uterine from 987 cases receiving IUD removal. The factors that included age, duration of IUD insertion, insertion at time of therapeutic abortion and types of IUD were investigated. And the statistical analysis was done. Results:(1) IUD trapped into the wall of uterine were positively related with age and duration of IUD insertion (P<0.05 and P<0.005 respectively). (2) The prevalence rate of IUD trapped into the wall of uterine was significantly higher for women inserted IUD at time of therapeutic abortion than that of women at time of non-menstrual period (P<0.005). (3) The prevalence rate was related with types of IUD inserted (P<0.005). IUD trapped was found to be the commonest in women with insertion of metal type. Conclusions:Higher possibility of IUD trapped should be considered for women who are old, carrying the device for long time, inserted at time of therapeutic abortion or inserted metal type of the device. During IUD removal, assistance with ultrasound monitoring is suggested to promote successful chance of IUD removal.
