颈上部肿块32例临床诊断分析

目的　探讨颈上部肿块的临床表现特点 ,诊断与鉴别诊断。方法　对 1990～ 1999年收治的以颈上部肿块为首发症状而就诊的病例资料 3 2例 ,进行回顾性分析。结果　 3 2例患者中男性 18例 ,女性 14例 ,年龄 3月～70岁。其中颈深上部淋巴结炎 15例 ,4 6.9% ( 15 / 3 2 ) ,恶性肿瘤或转移性癌 9例 ,2 8.1% ( 9/ 3 2 ) ,良性肿瘤 8例 ,2 5 % ( 8/ 3 2 )。结论　临床上因口腔和口周病变 ,感染或恶性肿瘤等均可引起头颈部淋巴部异常 ,本组 3 2例就诊时均为颈上部肿块为主要特征 ,少数为罕见病例 ,有的病例反复活检才得到确诊。因而对上颈部肿大淋巴结及肿块除常规检查外 ,仔细寻找原发灶进行鉴别诊断具有重要的临床意义。

AN ANALYSIS OF CLINICAL DIAGNOSIS OF THIRTY-TWO CASES OF GROWTH IN THE UPPER-NECK REGION

Objective To investigate the characteristics of the clinical features,diagnosis and differential diagnosis of the uper-neck growths. Methods materials from 1990 to 1999of thirty-two cases admitted,in whom the growth in the upper-neck region was the first complaint were analyzde retrospectively. Results among the 32 cases,18 were males and 14 were females.Aged from 3 months to 70 years.15 cases were diagnosed as lymphadenitis in the deep upper-neck region,46%(15/32);9 were malignant tumors or metastaltic carcinomas,28.1%(9/32)and 8 were benign tumors 25%(8/32). Conclusion any diseases,infection or malignant tumors in the mouth and peri-oral region may cause abnormal alternation of the lymphatic nodes.All the 32 cases in the present article had taken feature of mass-growing in the upper-neck region as their chief presentation when they first sought medical consultation.Some of the cases were rather clinically rare and in some cases repeated biopsies had been taken confirmed diagnoses were established.It is concluded,the enlarged lymphatic glands and growths occur in the upper-neck region,meticulous searches for their original foci for further differential diagnosis,in addition to the traditional routine examinations,are clinically significant and are of utmost importance.
