盐酸奥洛他定联合卡介菌多糖核酸对慢性自发性荨麻疹的疗效观察

摘    要: 目的: 探讨盐酸奥洛他定联合卡介菌多糖核酸对慢性自发性荨麻疹的疗效观察。方法:将90例慢性自发性荨麻疹患者随机分成观察组47例和对照组43例,观察组采用口服盐酸奥洛他定片联合肌内注射卡介菌多糖核酸注射液治疗,疗程为12周。对照组单纯口服盐酸奥洛他定片,用法、用量及疗程同观察组。采用荨麻疹活动度评分对两组患者治疗效果进行评估。经流式细胞仪检测两组患者治疗前后外周血T淋巴细胞亚群的含量变化,同时与45例健康体检者进行对照。结果:治疗12周后,观察组和对照组有效率分别为87.23%、69.77%,差异有统计学意义(X2=4.11,P=0.043)。12周时观察组外周血  CD3+, CD4+T淋巴细胞含量及CD4+/CD8+比值上升,与治疗前及对照组相比,差异有统计学意义(与治疗前比较,t分别为9.79、38.00、12.28,均P<0.01;与对照组比较,t分别为8.99、33.00、14.04,均P<0.01)。结论:盐酸奥洛他定联合卡介菌多糖核酸可调节慢性自发性荨麻疹患者T淋巴细胞亚群趋于稳态,从而达到治疗目的,二者联合用药安全有效。

Abstract:Objective: To investigate the clinical efficacy of Olopatadine hydrochloride combined with BCG polysaccharide and nucleic acid on patients with chronic spontaneous urticaria. Methods: Ninety patients with chronic spontaneous urticaria were randomly divided into the treatment group (47 cases) and the control group (43 cases). The treatment group was treated with oral olopatadine hydrochloride and intramuscular injection of BCG polysaccharide and nucleic acid. The control group was treated only with olopatadine hydrochloride. Urticaria activity score (UAS) was used to evaluate the clinical efficacy of the two groups. The changes of T lymphocyte subsets in peripheral blood of the two groups before and after treatment were detected by flow cytometry, and compared with 45 healthy subjects. Results:The 12 week effective rates of the treatment group and the control group were 87.23% and 69.77% respectively, and the difference was statistically significant ( X2=4.11, P=0.043). The 12 week levels of   CD3+, CD4+ T lymphocytes and the ratio of  CD4+/CD8+ in the treatment group were significantly higher than those before treatment and the control group(Compared with those before treatment:t=9.79,38.00,12.28,respectively, all P<0.01. Compared with the control group:t=8.99,33.00,14.04,respectively,P<0.01).Conclusion:Olopatadine hydrochloride combined with BCG polysaccharide nucleic acid can achieve the purpose of treatment by regulating the steady of T lymphocyte subsets in patients with chronic spontaneous urticaria. The combination of the two drugs is safe and effective.
