Neonatal convulsion of different causes:retrospective analysis of clinical features

Objective:To explore the clinical features of different causes of neonatal convulsion.Methods:The clinical data of 109 children with convulsion hospitalized in the Department of Neonatology,People's Hospital of Xinjiang Uygur Autonomous Region were retrospectively analyzed from January 2013 to September 2019.Results:The most common cause of neonatal convulsion was unknown cause(33.94%),followed by hypoxic-ischemic encephalopathy(HIE,20.18%)and intracranial infection(14.68%);HIE-induced neonatal convulsions mostly occurred within 3 days after birth,and unexplained neonatal convulsions could occur at any time of neonatal period;the most common type of seizures was mini seizures(41.28%).Phenobarbital had no effect on neonatal convulsion caused by genetic metabolic diseases,and its curative effect on neonatal convulsion caused by other causes was more than 50.00%.The positive rates of HIE,intracranial infection,cerebral infarction and intracranial hemorrhage were 66.67%,40.00%and 100.00%,respectively.The positive rate of EEG was 55.00%,and 5 cases of gene test were positive.Conclusion:In addition to unexplained neonatal convulsion,HIE is the most common cause of HIE,which has a variety of seizure forms,and the most common type is mini seizures.HIE,intracranial hemorrhage and cerebral infarction were the most common causes of convulsion in the first 3 days after birth.Brain magnetic resonance imaging can help to diagnose the cause of disease,and genetic examination can further clarify the cause of disease.Phenobarbital is not effective in the treatment of genetic metabolic diseases.
