Ovarian Brenner tumor:a clinicopathological analysis of 24 cases

Purpose To explore the clinicopathological characteristics,treatment and prognosis of ovarian Brenner tumor(BT),and to improve the understanding of ovarian BT.Methods Combined with literature review 24 cases of ovarian BT were studied in Wuxi Maternal and Child Health Hospital Affiliated to Nanjing Medical University from March 2013 to March 2020.Clinical data,histopathological characteristics and immunohistochemical phenotype were analyzed.Results Sixteen cases(66.7%)were benign BT,7 cases(29.2%)were borderline BT,and 1 case(4.1%)was malignant BT.Patients with benign and borderline BT were mostly found by accident or by medical examination.The malignant BT was treated with abdominal pain.Grossly,both benign and borderline BT were all gray-red cystic solid masses with gray-yellow cut surfaces,slightly braided,and slightly tough texture.The malignant BT was a gray-red cyst.The solid surface of the cut surface was colorful,and the solid area was gray-yellow granules.The cystic area contained bloody fluid.Microscopically,benign BT showed a dense fibrous interstitium with hyperplastic transitional epithelial nests,and the cells had no obvious atypia.The borderline BT presented cystic and nodular areas,and the cyst was lined with multi-layer transitional epithelium,epithelial cell hyperplasia with atypical,no interstitial infiltration.Malignant BT was a papillary tumor with solid hyperplasia,increased cellularity,with heteromorphism,increased mitosis,and interstitial invasion,accompanied by benign or borderline BT component.Immunophenotypically,CK7,p63 and GATA3 were positive in the tumor cells.CK20,C-erbB-2 and PAX8 were negative.3 cases were positive for WT-1.ER and PR were positive in malignant BT,ER and PR were scattered positive in 1 case of borderline BT.ER and PR were negative in other BT.Ki-67 proliferation index in malignant BT was 40%,borderline BT was 10%to 20%,and benign BT was<5%.Conclusion Ovarian BT is a clinically rare tumor that exhibits specific histopathological and immunopathological characteristics and is mostly mixed with other epithelial and germ cell tumors.Malignant BT needs more cases for systematic and meta-analysis.
